All of your membership information will be kept in your electronic records, meaning there is no need for you to worry about bringing an ID card or group or membership number with you! You can begin receiving dental care the day you sign up, and your plan will renew each year at the same time. Please remember that the Direct Care Plan is a dental discount plan, NOT dental insurance. You cannot combine it with any other dental/medical insurance, or with any other discounts. It is valid only at our office. Our discount plan is non-refundable and is subject to change yearly. You must remain a member of the Direct Care Plan throughout your entire treatment, and all members on your plan must live at the same address.
